One Health & Care
New Service Announced

S&SHIS is pleased to announce the onboarding of a new service commencing 1st December – One Health and Care (OH&C).
What is OH&C?
OH&C is an integrated electronic shared care record that securely brings together patients’ health information from multiple providers.
It ensures that healthcare professionals have access to the most up-to-date and relevant information, leading to better coordination of care and improved patient outcomes.
With OH&C, clinicians gain a comprehensive view of a patient’s health history, including social care and mental health data.
This richer, consolidated information enables safer, more informed decisions and reduces time spent chasing details freeing up more time for patient care.
Customers include:
- Shropshire, Telford and Wrekin Integrated Care Board
- Black Country Integrated Care Board
- Staffordshire and Stoke-on-Trent Integrated Care Board
The service covers:
- NHS Trusts
- GP Practices
- Community Services
- Social Care Teams
- Mental Health Services
